1. Shipping fee is not included.
2. Customers may select either one options below for the product collection:

• Collect product at one of the Health Check Centers during office hours within 7 working days (Appointment is required & expired case is not refundable); or
• Collect product by courier service with payment by receiver within 7 working days

3. The actual shipping date will be subject to stock availability at center.
4. All purchased product(s) cannot be used in conjunction with any other promotional offers such as Credit Card Year Round offers, vouchers, gift vouchers or redemption programs.
5. All privileges/ gifts are not convertible to cash and cannot be replaced with other services/ products.
6. Amendment and cancellation are not allowed once the payment has completed.
7. If purchaser is not product recipient, the purchaser must print out and complete the Authorization Letter in the email attached with signature. The product recipient must present the original signed Authorization Letter to the scheduled health check centre to redeem the product.
8. After the transaction is completed, MediFast Hong Kong Customer Service Team will contact the purchasing customer via email eshop@medifasthk.com or phone call (+852 2272 8222) in 2 working days.
9. Please contact our Customer Service Team at (+852 2272 8222) during office hours if any changes of your appointment is needed.
10. Should the product(s) or service(s) is/are provided by any third party, Medifast Hong Kong will not be liable for any issues include shortage of stock, infringement, misappropriation or violation of another’s right (including but not limited to intellectual property), loss or damage due to the mailing service if applicable.
11. MediFast (Hong Kong) Limited reserves the final rights to amend the offer and its terms and conditions without prior notification.
