1. This service must be completed within 90 days upon the payment date, otherwise the order will be expired and is not refundable.
  2. All purchased service(s) cannot be used in conjunction with any other promotional offers such as Credit Card Year Round offers, vouchers, gift vouchers or redemption programs.
  3. All privileges/ gifts are not convertible to cash and cannot be replaced with other services/ products.
  4. Amendment and cancellation are not allowed once the payment has completed.
  5. If purchaser is not service recipient, the purchaser must print out and complete the Authorization Letter in the email attached with signature. The service recipient must present the original signed Authorization Letter to the scheduled health check centre to redeem the service.
